Packages either include monthly or fortnightly 1 : 1 nurse-led monitoring and guidance, with flexibility to adjust your level of support over time.

Coordinated care, without the fragmentation: CaptureCare helps you stay on top of your health — without needing to navigate multiple providers on your own. Depending on your needs, we can coordinate and support access to:
– Blood test referrals and advanced biomarker testing
– DEXA bone scans and age-appropriate screening

– Lifestyle and behaviour change support, including coaching and guidance. Your results can be shared with your GP, ensuring continuity of care while reducing the need to manage everything yourself.
