Step 2: Connect your data and set SMART health goals

We use secure software that links your health data securely to our system. We help you: Set up devices, set health goals, take accurate readings, and understand your health data

Step 3: Get ongoing nurse support and guidance

Our experienced nurses check in via Telehealth and keep track of your key health metrics. You get regular health summary reports, early alerts, and human care that makes a difference.

All packages include nurse-led monitoring and guidance, with options for:

– monthly or fortnightly telehealth sessions with your dedicated CaptureCare nurse

– more frequent check-ins and deeper support as your needs change

– access to women’s health specialists in higher-level packages, including areas such as perimenopause and menopause, sexual health, mental health, and other midlife health priorities

Your package can evolve over time. Some women start with a lighter level of support and step up as they want more oversight, education, or specialist input. Others prefer comprehensive support from the outset.
