The CaptureCare Preventative Remote Patient Monitoring (PRPM) process

Step 1: Take our stroke risk assessment test

Begin with our stroke risk assessment, developed by the National Institute of Stroke and Applied Neurosciences and endorsed by trusted institutions like Ballarat Health Services, the World Stroke Organisation, and the Menzies Institute for Medical Research at the University of Tasmania.

This quick and simple test identifies your risk factors and provides valuable insights into your health.

Step 2: Identify your Needs and Defining Goals

Based on your assessment results and current health concerns, we’ll help you determine if the PRPM program is right for you. Together, we’ll set clear goals for the program, such as:

  • Reducing your risk of serious medical events
  • Improving your overall health
  • Enhancing communication with your GP and medical specialists

Step 3: Enrol in the PRPM program

Before you enrol, we’ll guide you through the PRPM process to ensure you understand how it works. Once you provide informed consent:

  • CaptureCare will securely collect and transmit your health data to the PRPM platform.
  • If you approve, we can share regular health summary reports with your healthcare providers, helping them deliver better care.

Step 4: Onboarding and Device Setup

We’ll provide you with the necessary monitoring devices and clear instructions on how to:

  • Use the devices effectively
  • Take accurate measurements
  • View your data
  • Charge and maintain your devices

Our technical support team will be available to assist you with any device issues. Devices may include: Smartwatches, Scales, Blood Pressure (BP) and ECG wireless devices, Sleep mats, Smart toilet seats.

Step 5: Ongoing Monitoring and Support

Our nurses will:

  • Review your health data daily to identify trends, patterns, or concerns
  • Ensure you’re collecting data correctly and consistently
  • Provide timely interventions when needed to help you stay on track with your health goals

Step 6: Continuous Care and Improvement

We’ll stay in touch regularly to:

  • Answer your questions and address concerns
  • Gather your feedback to improve the PRPM program
  • Provide you with updated or upgraded monitoring devices if required
  • With your approval, we’ll continue sharing health summary reports with your healthcare providers to ensure they’re informed and able to support your care effectively.
